Requirements for Thai Visa Application

To obtain a medical visa for Thailand, you’ll need to prepare and submit the following documents:

1. Passport:

    • Must be valid for at least 6 months.
    • Include the previous two passports.
    • Provide a GD copy (English translated and notarized) if your previous passport was lost or missing.
    • Include a marriage certificate if your spouse’s name isn’t on the passport.
    • An affidavit if family member names differ from the father/mother/husband/wife.

2. Photographs:

    • Two recent passport-sized photos (3.5×4.5 cm).
    • Must have a white background and be printed on matt paper.

3. Bank Statement and Solvency:

    • Bank statement for the last 6 months, including regular transactions (at least 5 per month).
    • Bank solvency letter with the same name as your passport, contact number, and a minimum balance of BDT 60,000 for a single person or BDT 1,20,000 for a family.

4. Trade License:

    • For business owners, provide an up-to-date trade license (translated and notarized).
    • Include a company letterhead with an active contact number and address and a business card of the applicant.

5. No Objection Certificate (NOC):

    • For service holders, provide a salary certificate, pay slip, or salary account statement.
    • Include a photocopy of your office ID card.

6. GO Letter:

    • For government employees, provide a GO letter.
    • Include a retirement copy if retired.

7. School ID Card:

    • For students, provide a valid school ID card with the school’s contact number and an NOC from the school on official letterhead.

8. Medical Documents:

    • Recent medical reports or a doctor’s referral letter (clear scan copy).

9. Air Ticket:

    • Provide a round-trip air ticket or e-ticket booking copy.

10. Academic Certificate:

    • Include your last academic certificate if unemployed.

11. Bank Account:

    • A bank account is required if you are a servant.

12. Contact Numbers:

    • Provide two active phone numbers of the visa client.

19. Guarantor Details:

    • One active guarantor’s phone number, full name, and address for the visa form.
